What should you know about a goiter lump on the neck?

A goiter is an enlarged thyroid, the butterfly-shaped gland at the front of the lower neck. Enlargement can be smooth or nodular, and thyroid function may be low, high, or within the usual range. A lump elsewhere in the neck may instead involve a lymph node, salivary gland, cyst, or inflamed tissue. A neck lump alone does not establish a cancer diagnosis. Thyroid cancer is one possibility among several, while head and neck cancer or lymphoma may be considered when examination findings point beyond the thyroid.

Arrange an appointment with a doctor, especially if the swelling is new, growing, firm, or persistent. The visit may include feeling the thyroid and nearby nodes, checking the mouth and throat, and reviewing symptoms such as voice change, swallowing difficulty, heat or cold intolerance, and palpitations. Tests can include thyroid blood tests, an ultrasound, and sometimes a needle sample of a thyroid nodule. Do not squeeze the lump or start thyroid medicine without advice. If cancer is suspected, the next step is guided by the examination and test findings; surgery is not automatically required.

Seek urgent medical help if the swelling makes breathing difficult, swallowing suddenly unsafe, or causes rapidly worsening throat pressure. Request prompt assessment for a lump that continues to enlarge, becomes hard or fixed, or comes with persistent hoarseness, unexplained weight change, night sweats, or swollen nodes. These features do not prove cancer, but they should not be left unevaluated.

Ask where the lump appears to arise, whether your thyroid function needs testing, and whether ultrasound is appropriate. You can also ask what would make a needle biopsy useful, how quickly results are expected, and whether a thyroid specialist or surgeon is needed.
