What should you know about goiter caused by hyperthyroidism?

The thyroid sits low in the front of the neck. When it becomes enlarged, you may notice swelling, tight collars, a visible fullness or a pressure feeling. Hyperthyroidism means the gland is making more thyroid hormone than the body needs. A goiter may be spread evenly through the gland or may contain one or more enlarged areas. Graves’ disease is one possible cause, but thyroid inflammation and hormone-producing nodules can also lead to an overactive thyroid.

The hormone excess can cause a fast or forceful heartbeat, feeling anxious, increased sweating, heat intolerance, difficulty sleeping, diarrhoea and weight loss. These symptoms can point to hyperthyroidism, but a neck enlargement does not identify the cause by itself.

Arrange an appointment with a doctor for a neck examination and thyroid blood tests. Depending on the findings, an ultrasound or another thyroid scan may help show whether the enlargement is diffuse or nodular and guide further evaluation. Treatment is chosen according to the cause, hormone results, symptoms, overall health and pregnancy plans. Options may include medicine to reduce hormone production, medicine for troublesome palpitations, radioactive iodine or surgery. Do not start iodine supplements or thyroid medicines without medical advice, because they can alter thyroid activity.

Until you are assessed, keep a record of the swelling and any changes in heartbeat, bowel habits, sweating, sleep or weight. Tell the clinician about medicines and supplements, including products marketed for energy or weight loss.

Seek urgent medical help if the swelling makes breathing or swallowing difficult, or if you develop chest pain, fainting, severe weakness, confusion, fever or a very fast or irregular heartbeat. These symptoms can signal a serious effect of excess thyroid hormone or pressure from the enlarged gland. A rapidly changing neck swelling or new hoarseness also needs prompt clinical review.

Ask what is causing the goiter, which thyroid tests are needed, whether imaging is appropriate and how your symptoms should be monitored. You can also ask how each treatment may affect heart rate, fertility, pregnancy, other medicines and follow-up care.
