GERD and hoarseness: what to know

Reflux reaching the throat can irritate the vocal folds, causing a rough, weak, or strained voice, frequent throat clearing, cough, or a lump sensation. Symptoms may be more noticeable on waking or after meals, but this pattern does not establish the cause. A common cold, chronic cough, thyroid problems, voice overuse, smoking, and changes affecting the throat, lungs, or vocal folds can also produce hoarseness. An examination is useful when the voice does not return to its usual quality.

Drink fluids regularly, rest your voice, and avoid shouting or whispering, which can strain the vocal folds. Do not smoke, and limit personal reflux triggers. Eat smaller meals and remain upright after eating if lying down worsens symptoms. Avoid repeatedly clearing your throat; sip water instead. A clinician may review reflux treatment and examine the throat or arrange an ear, nose, and throat assessment when needed.

Book a medical review for hoarseness that persists, returns often, or comes with a neck lump, persistent cough, pain when swallowing, difficulty swallowing, unexplained weight loss, or coughing blood. Seek urgent help for breathing difficulty, noisy breathing, or rapidly worsening swelling. These features need assessment rather than treatment based on reflux alone.
