What may be behind a calf cramp

The gastrocnemius is the prominent muscle at the back of the lower leg. During a cramp it contracts without your control, making the calf feel hard and painful. Triggers can include strenuous activity, prolonged standing, muscle strain, dehydration or changes in the body’s mineral balance. Recurrent cramps can also occur alongside conditions such as peripheral arterial disease, chronic kidney disease, hypothyroidism or vitamin D deficiency. Sarcopenia, which involves loss of muscle mass and strength, may affect muscle resilience.

What to do during and after it

Stop the activity and gently straighten the leg. Pull the toes towards you while keeping the knee as comfortable as possible, or stand and place weight gradually through the foot if safe. Light massage and warmth may relax the muscle; avoid forceful stretching. Drink fluids if you may be dehydrated. Once the pain settles, gentle calf and ankle movement can restore comfort. Record when cramps occur, what exercise preceded them and any new medicines.

When to seek care

Arrange a review if cramps are frequent, disrupt sleep, last unusually long, or began after a medicine change. Seek urgent assessment for a swollen, red or hot calf, sudden breathlessness, chest pain, a cold or pale foot, new weakness, or pain after a significant injury. These features may need a different explanation from an ordinary muscle cramp.
