The short answer

A cramp can make a calf, foot, thigh, hand, or another muscle feel hard and locked for a short time. The muscle may remain tender afterwards. Keeping a brief record of timing, body area, activity, fluids, and medicines can help a clinician look for a pattern.

What frequent cramps can mean

Cramps after sweating, vomiting, diarrhoea, prolonged exercise, or not drinking enough may occur with dehydration. A muscle strain or an abrupt change in activity can also irritate a muscle. Some medicines and changes in body salts can contribute, so do not stop a prescribed medicine without advice.

Recurring cramps can also occur alongside conditions affecting nerves, blood flow, muscles, hormones, or kidney function. Peripheral arterial disease may cause leg discomfort with walking, while hypothyroidism can be associated with muscle symptoms. Vitamin D deficiency, sarcopenia, and chronic kidney disease are other possible health contexts, but a cramp alone does not identify its cause.

What to do during and between cramps

Stop the activity and gently stretch the affected muscle. For a calf cramp, straighten the leg and draw the toes towards your face, or stand and place weight on the leg if that feels safe. Gentle massage and a warm shower may help the muscle relax; use cold later if soreness follows activity.

Drink fluids regularly, particularly around exercise or hot weather, and replace meals rather than taking mineral supplements on your own. Warm up before unfamiliar exercise, increase workload gradually, and avoid holding one position for a long time. Arrange a medical review if cramps keep returning, disturb sleep, follow a medicine change, or limit walking or daily tasks.

When to seek medical care

Seek urgent care for a cramp with a cold, pale, or blue limb; sudden weakness or loss of feeling; severe swelling; chest pain; trouble breathing; fainting; or dark urine after intense exertion. These signs can point to a problem beyond an ordinary muscle spasm.

Contact a clinician soon when cramps are new and persistent, occur throughout the body, become progressively more painful, or come with fever, vomiting, diarrhoea, marked thirst, muscle weakness, or reduced urination. A clinician may examine your muscles and circulation and review medicines and relevant blood tests.
