What recurring hoarseness can mean

Hoarseness may sound raspy, breathy, weak or rough when the vocal cords cannot vibrate normally. A common cold, shouting, singing, frequent throat clearing, smoke or dry air can irritate them. Acid reflux may reach the throat and cause morning hoarseness, a lump sensation or repeated clearing.

Chronic cough can keep the throat irritated. Hypothyroidism may be associated with a deeper or rougher voice. Persistent hoarseness can also arise from a vocal-cord lesion or, less commonly, a cancer affecting the throat, lung or thyroid. Hoarseness alone does not identify cancer, and most short-lived episodes have a less serious explanation.

What to do next

Rest your voice, use a comfortable speaking volume, drink fluids and avoid shouting, whispering and smoke exposure. Treat a cold as advised and avoid repeated forceful throat clearing; sipping water can be gentler. Note whether the change follows voice use, infection, meals or lying down.

A clinician may examine your mouth and neck, review medicines and smoking exposure, and refer you for a vocal-cord examination if the change persists or keeps returning. Do not start reflux treatment indefinitely without checking the cause.

When to seek care

Get emergency help for sudden trouble breathing, noisy breathing, inability to swallow saliva or rapidly increasing neck swelling. Arrange prompt review for coughing blood, unexplained weight loss, a neck lump, persistent ear pain, swallowing difficulty or hoarseness that does not settle. People who use their voice professionally may need earlier advice because voice damage can affect work.
