Freckles on the vulva: what to know

Freckles are linked to pigment production and may become more noticeable after sun exposure, although vulval freckles are not usually sun-exposed. Other benign pigment marks, irritation, inflammation, or a mole can resemble a freckle. Appearance, history, and change over time help guide the assessment; do not try to diagnose a genital mark from a photograph alone.

Arrange a routine appointment if the mark is persistent, unfamiliar, or worrying you. Avoid bleaching products, acids, home freezing kits, and unapproved lightening creams on vulval skin. Use gentle, fragrance-free cleansing and reduce rubbing from tight clothing. Treatments such as IPL Photofacial or Picosecond Laser Treatment may be discussed only after diagnosis and a clinician confirms that treatment is appropriate for this sensitive area.

Seek prompt medical advice for rapid enlargement, several colours, an uneven border, a firm or raised area, repeated bleeding, ulceration, persistent pain, or marked itching. Also arrange examination when a spot changes after pregnancy, hormonal treatment, or repeated irritation. These features do not identify a diagnosis by themselves, but they make direct assessment more important.

Ask whether the mark has features of a freckle, mole, irritation, or another pigment condition; whether examination with magnification is needed; what changes to photograph or record; and whether any cosmetic treatment could irritate vulval skin.
