Freckles on scars: what to know

A scar can remain more noticeable when nearby skin produces extra melanin. Sunlight may darken both the scar and small brown macules around it. Healing inflammation, friction, or a darker mark left after irritation can also make the area look freckled. Freckles themselves are usually flat and evenly coloured, while scar tissue may feel firmer, smoother, or raised.

A photograph taken in similar lighting can help you notice gradual change. A clinician may use magnification to distinguish freckles from a mole, post-inflammatory pigmentation, or another lesion.

Cover the scar when practical and use broad-spectrum sunscreen on exposed skin, following the product directions and reapplying after sweating or swimming. Avoid picking, harsh scrubs, and unprescribed lightening products, because irritation can deepen pigmentation.
