Freckles on eyelids: what to know

The thin skin of the eyelids can develop freckles, also known as ephelides, after ultraviolet exposure. A freckle is often flat and fairly even in colour, while other marks can include a mole, lentigo, irritation-related pigment, or a lesion arising from the eyelid margin. Pigment on the lid or near the lashes can be difficult to judge in a mirror. Do not assume a mark is harmless because it is small or has been present for a while; comparison over time and professional examination are useful.

Use sunglasses that block ultraviolet light, a brimmed hat, and carefully applied sunscreen on surrounding skin, keeping products out of the eyes. Avoid rubbing the lids and do not apply acids, bleaching creams, or home devices close to the eye. A dermatologist or eye specialist should assess the spot before cosmetic treatment. IPL Photofacial and Picosecond Laser Treatment are not automatically suitable for eyelid skin; the location, eye protection, diagnosis, and device settings matter.

Arrange prompt assessment if the mark changes in size, shape, or colour; develops crusting, bleeding, ulceration, or persistent itch; causes lash loss; or sits on the eyelid edge and keeps irritating the eye. Seek urgent eye care for vision changes, significant swelling, severe pain, or an eye injury. Bring a clear photo showing how the mark has changed if one is available.

Ask whether the spot is on the eyelid skin or margin, whether it needs dermoscopic or eye examination, and how often it should be checked. If treatment is considered, ask who will provide eye protection, what temporary effects to expect, and which alternatives avoid the eye area.
