Freckles on the back of the hand: what to know

The backs of the hands receive frequent sun exposure and are easy to overlook when applying sunscreen. Freckles are flat pigment spots that may darken after sunlight and fade when exposure decreases. Their colour can range from light tan to brown. A freckle pattern may occur alongside other sun-related marks, but a darker, rough, scaly, or thicker patch may be a different type of lesion.

The location alone does not identify a spot. Compare the mark with nearby skin and with photographs taken earlier. A dermatologist can use magnification or dermoscopy to assess detail that is hard to judge at home.

Apply broad-spectrum sunscreen to the backs of the hands and reapply after washing or prolonged outdoor activity. UV-protective clothing can add coverage. Do not use strong lightening products, acids, or home freezing kits on an undiagnosed mark, because irritation may obscure its appearance. If the spots are a cosmetic concern after assessment, ask about IPL Photofacial or Picosecond Laser Treatment and the possibility of temporary redness or pigment shifts.

Book a review if a spot becomes asymmetrical, develops an irregular edge, gains multiple shades, enlarges, or becomes raised. Bleeding without injury, repeated crusting, persistent scaling, pain, or a non-healing sore also warrants assessment. A rapidly changing lesion should be prioritised, even if other freckles look unchanged.

Ask what type of pigmentation is present, whether a particular mark needs dermoscopy or sampling, and how often you should check your hands. Before cosmetic treatment, ask how the procedure interacts with your skin tone, recent tanning, medications, and any lesion that has not yet been diagnosed.
