What the change can mean

Freckles, also called ephelides, are small areas where pigment is more concentrated than in the surrounding skin. Pregnancy can make existing freckles look deeper in colour, and other facial pigmentation may appear at the same time. Ultraviolet exposure can add to this darkening, so the change may be more obvious after time outdoors. Freckles do not become a pregnancy complication simply because they look darker.

A broad, fairly even increase in pigmentation is different from one spot developing a new irregular pattern. A clinician may need to examine a mark that changes in shape, border, colour, or surface rather than assuming the change is pregnancy-related.

What you can do

Use broad-spectrum sunscreen on exposed skin, reapply it as directed on the product, and add shade, a wide-brimmed hat, or clothing that covers the area. Choose a gentle cleanser and moisturiser, and avoid picking or scrubbing freckles because irritation can leave pigmentation more noticeable.

Do not start a pigment-lightening cream, peel, or home device without checking its ingredients and suitability with your pregnancy clinician or dermatologist. Cosmetic light-based procedures are usually postponed during pregnancy while safety and the cause of the pigmentation are clarified. After delivery, a dermatologist can discuss options such as IPL Photofacial or Picosecond Laser Treatment if a spot remains bothersome.

When to seek care

Arrange a dermatology assessment if one freckle is changing noticeably, has an uneven border or several colours, becomes raised, repeatedly bleeds, forms a persistent crust, or develops ongoing itch or tenderness. Seek prompt medical advice for a rapidly changing or ulcerated spot. Take a clear photograph in good light and note when you first noticed the change; this can help the clinician compare it over time.
