Frank–Starling curve and heart failure: what to know

As more blood returns to a healthy heart, its muscle fibres stretch within a useful range and the next contraction can become stronger. This helps match pumping with venous return. In heart failure, the weakened heart may gain little extra pumping force from additional filling. Pressure can then build behind the affected chamber, contributing to breathlessness, cough, tiredness or swollen ankles. The curve is a physiology concept, not a home test and not a diagnosis by itself. Atrial fibrillation and hypertension can worsen the workload on the heart.

Arrange a clinical assessment if symptoms are new, persistent or changing. A clinician may review your history, examine your heart and lungs, check blood pressure and oxygen level, and arrange tests such as an ECG, blood tests or an echocardiogram. Take prescribed medicines as directed and do not change diuretics or fluid intake without advice. Record changes in breathlessness, exercise tolerance, sleep position, ankle swelling and weight trend if your care team has asked you to monitor it. A chronic disease review can help coordinate follow-up.

Seek emergency help for severe or rapidly worsening breathlessness, chest pressure, fainting, new confusion, blue or grey lips, or palpitations accompanied by collapse or marked weakness. Contact a clinician promptly if swelling is increasing, you are waking breathless, or ordinary activity has become noticeably harder. These warning signs need assessment because fluid build-up or an abnormal rhythm can deteriorate.

Ask which type of heart failure is being considered, what your examination and echocardiogram show, which symptoms should trigger a same-day call, and how to take or monitor each medicine. You can also ask whether an irregular rhythm, high blood pressure or another condition may be adding strain.
