What it involves

The joint at the base of the big toe helps the foot push off during running. Turf toe can irritate its capsule, ligaments, and nearby soft tissues, causing pain underneath or around the joint, swelling, bruising, stiffness, or difficulty pushing off. It may happen in one forceful incident or build through repeated loading in flexible football footwear. Toe pain can have other causes, including a fracture or joint wear, so persistent symptoms should be assessed. Hallux Rigidus is a different condition involving stiffness and wear at the same joint.

What to do now

Stop sprinting, kicking, and jumping if they increase pain. Elevate the foot and use a wrapped cold pack for short periods. Choose a stiff-soled, roomy shoe and avoid bending the big toe upward repeatedly. Do not force stretches or test the injury by repeatedly pushing off. A clinician may recommend taping, a protective insert, or Splinting, depending on stability and severity. Physiotherapy can later address movement, foot strength, balance, and a gradual return to football.

When to seek care

Seek prompt assessment if the toe is crooked, numb, cold, severely swollen, or impossible to move after an injury. Urgent care is appropriate if you cannot bear weight, have an open wound, or develop spreading redness and fever. Arrange a review when pain persists, push-off remains weak, or the joint repeatedly catches or feels unstable.
