Foot swelling after a C-section: what to know

Short answer
Foot swelling after a C-section can follow pregnancy fluid shifts, intravenous fluids, and reduced movement, but one-sided swelling or breathing symptoms need urgent review.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Often seen
- Mild swelling around both feet and ankles
- Do not ignore
- One-sided painful swelling or new breathing symptoms
Why swelling may happen
After a caesarean birth, extra fluid from pregnancy and treatment may move into the tissues around the feet and ankles. Spending more time in bed can also slow circulation. Mild swelling on both sides may improve as you move safely and your body clears excess fluid. A swollen, tender, warm, or red leg is different and may indicate a clot. High blood pressure complications can also present after birth, including after a C-section; Pre-eclampsia can involve headache, vision changes, or upper abdominal pain.
Supporting recovery
Follow the mobility plan from your surgical team. Short, gentle walks and ankle pumps can help circulation when permitted. Rest with your feet raised, avoid tight footwear, and do not massage a painful or one-sided swollen leg. Take prescribed medicines as directed and keep follow-up appointments. Postnatal Physiotherapy may help with safe movement and recovery when your clinician recommends it.
When to seek care
Get urgent medical help for chest pain, sudden breathlessness, coughing blood, faintness, or a leg that becomes painful and swollen on one side. Contact your maternity team promptly for severe headache, visual disturbance, upper abdominal pain, rapidly increasing swelling, or feeling very unwell. Also report fever, worsening wound pain, or discharge from the incision.
Questions for your doctor
Is my swelling expected after surgery and fluids? When can I increase walking? Do I need a blood pressure check, clot assessment, or review of my medicines?
