Foot pain from the big toe to the arch: what to know

Pain along this line can occur when the big-toe joint is stiff, making the foot roll differently during a step. Hallux rigidus can limit upward toe movement; a bunion can shift pressure toward the inner foot; and a hammer toe can change the balance of the forefoot. Flat feet may add strain along the inside border. Look for swelling, a visible bump, reduced toe movement, or pain that appears during push-off.

Choose shoes with a wide toe box and enough depth, and avoid footwear that presses on the big-toe joint. Reduce jumping or long walks if push-off is painful. Gentle movement of the toe is reasonable only within a comfortable range. A foot and ankle clinician can assess alignment and joint motion; custom orthotics may help some pressure patterns but should be fitted to your needs.

Arrange an examination if the toe becomes progressively stiff, the pain affects walking, or a bump or swelling keeps rubbing in shoes. Seek urgent care after injury when you cannot bear weight, or with severe swelling, deformity, numbness, or a hot red joint with fever.

Ask which joint or tendon is involved, whether toe motion is restricted, and how footwear should change. Ask whether exercises, orthotics, or another treatment could reduce pressure during push-off.
