What should you know about focal tonic seizure?

Focal means the electrical disturbance starts in a limited brain region. Tonic describes muscles becoming stiff rather than repeatedly jerking. The arm, leg, face, or one side of the body may be involved. Some people remain able to respond; others become confused or lose awareness as the seizure spreads. A seizure can have several possible triggers, including low blood sugar, alcohol withdrawal, infection, head injury, or a seizure disorder. A single event does not by itself establish the cause.

Afterward, tiredness, headache, muscle soreness, or temporary confusion can occur. The person may not remember the episode clearly.

Keep the person away from traffic, water, sharp edges, and heat. Cushion the head, loosen tight clothing around the neck, and note when the stiffening starts. Do not restrain the movements, put anything in the mouth, or give food, drink, or medicine until full alertness returns. When the seizure ends, place the person on their side and stay with them while they recover. A witnessed description or video, if safely obtained, can help a clinician assess the event.

Call emergency services if this is a first suspected seizure, the episode continues, another seizure follows before recovery, breathing is difficult, or the person is injured, pregnant, or remains unusually unresponsive. Urgent assessment is also needed when fever, severe headache, stiff neck, or a recent illness raises concern for meningitis. Ask what features suggest a focal seizure, whether blood tests or brain-wave testing are needed, how to use any prescribed rescue medicine, and which activities should wait until the cause is clearer. Mention medicines, alcohol use, missed meals, sleep loss, fever, and any previous episodes.
