Fluid retention after birth: what to know

Pregnancy changes how the body handles salt, water, and blood flow. Extra fluid given during labour or a caesarean section can add to swelling, and hormonal changes after delivery alter how that fluid is released. Swelling may be more noticeable in the feet, ankles, fingers, or face and can take time to settle.

Postpartum swelling is not always harmless. High blood pressure after birth, including pre-eclampsia, can occur even when pregnancy has ended. A clot can cause pain and swelling in one leg, while chest symptoms may indicate a clot that has travelled to the lungs.

Rest with your legs supported, change position regularly, wear comfortable footwear, and drink normally unless your clinician has advised a fluid restriction. Gentle walking may support circulation when you are able, but do not massage a painful or swollen leg. Check whether swelling is improving and attend postpartum blood-pressure or wound reviews.

Tell your maternity team about headaches, visual changes, breathlessness, or swelling that is increasing. Mention whether you had a caesarean section, received medicines such as oxytocin, or have a history of blood-pressure problems.

Seek emergency help for chest pain, sudden breathlessness, coughing blood, collapse, or severe difficulty breathing. Contact urgent maternity or medical services for a severe headache, vision changes, pain under the ribs, sudden face or hand swelling, very high blood pressure, or one leg that is hot, red, painful, and more swollen than the other.
