Why the voice changes
Inflammation around the vocal cords can make the voice rough, weak, breathy, or lower than usual. Frequent coughing, throat clearing, dry air, and dehydration can add strain while you are ill. A common cold can produce a similar change. Acid reflux, an underactive thyroid, or a chronic cough can also affect the voice. Hoarseness alone does not identify a serious disease; the duration, associated symptoms, smoking history, and examination findings guide assessment.
Protecting your voice
Rest your voice by speaking gently and only when needed; whispering can also strain the vocal cords. Drink fluids, use humidified air if it feels soothing, and avoid smoke and other irritants. Try not to repeatedly clear your throat, as a sip of water may be kinder. Treat a cough according to professional advice. Do not start antibiotics solely for hoarseness, because many respiratory illnesses are viral and antibiotics do not treat viruses.
When hoarseness needs review
Seek urgent help if hoarseness comes with severe breathing difficulty, noisy breathing, inability to swallow saliva, sudden facial weakness, or swelling of the mouth or throat. Arrange a medical review if the voice does not recover after the illness, keeps returning, or comes with coughing blood, unexplained weight loss, a neck lump, persistent pain, or difficulty swallowing. A clinician may examine the throat and recommend an ear, nose, and throat assessment when needed.
