Flu but no fever: what it can mean and what to do

What it may mean
Not everyone with influenza develops a fever, and some people may have a fever that was missed between readings. Older adults and people with reduced immune responses may show less of a temperature rise. Other respiratory infections can look similar. A common cold often builds more gradually and may cause a runny or blocked nose, while flu more often brings a sudden, heavy feeling of illness. COVID-19 and acute bronchitis can also cause cough, fatigue, or body aches. These patterns overlap, so symptoms cannot identify the infection with certainty.
Watch the direction of the illness. Gradual improvement supports home care, while worsening breathlessness, dehydration, confusion, or severe weakness needs prompt attention. A cough that lingers after the main illness can occur, but new deterioration after initial improvement deserves medical review.
What to do at home
Rest, drink fluids regularly, and choose small, easy meals if your appetite is low. Stay home while you feel unwell and improve ventilation around other people. Cough into your elbow or a tissue, wash your hands, and avoid close contact with people who could become seriously ill. Follow the label for any over-the-counter medicine, and check with a pharmacist or clinician if you are pregnant, have a long-term condition, take regular medicines, or are treating a child. Do not combine products that contain the same pain-relieving ingredient.
If symptoms started recently and you are at higher risk of complications, contact a clinician promptly because antiviral treatment may be time-sensitive. Testing may help distinguish influenza from COVID-19 when the result would change your care or precautions.
When to seek care
Seek urgent medical help for trouble breathing, chest pain or pressure, blue or grey lips, new confusion, fainting, inability to stay awake, or signs of severe dehydration such as very little urine. For a child, urgent assessment is also needed for marked difficulty breathing, unusual drowsiness, or a child who is not drinking.
Arrange medical advice if symptoms are worsening, you cannot keep fluids down, you have a serious long-term condition, or you are pregnant. Also seek review if you start to improve and then become clearly worse, or if a cough, weakness, or shortness of breath is not settling. A clinician can assess whether influenza, COVID-19, pneumonia, or another illness is more likely.
