What chills without fever may mean

Cold surroundings, damp clothing, tiredness, anxiety and a strong emotional response can all make you shiver. Not eating for a long time may contribute through low blood sugar, especially if you also feel sweaty, weak or light-headed. An infection can begin with chills before temperature changes are recorded. Influenza, dengue fever, malaria, a urinary tract infection or kidney infection may bring other clues such as aches, cough, burning urine, back pain, vomiting or marked fatigue. Chills alone cannot identify the cause.

Move to a comfortable, dry environment and add light layers. Sip fluids and eat a small balanced snack if you have missed a meal and can swallow normally. Check your temperature with a reliable thermometer and record when the chills start, what you were doing, and any new symptoms. Arrange a clinical review if episodes recur, last beyond a short period, or follow travel to a malaria-risk area.

When to seek care

Seek urgent help for confusion, fainting, severe weakness, trouble breathing, blue or unusually pale skin, a seizure, severe headache with a stiff neck, or rapidly worsening illness. Get prompt medical advice for chills with persistent vomiting, severe abdominal or back pain, painful urination, a rash, or recent travel where malaria is possible. Do not wait for a fever if you feel seriously unwell; sepsis can cause a dangerous deterioration and needs emergency care.
