Flat Feet and Leg Symptoms: What Should You Know?

Short answer
Flat feet can change how forces travel through the ankles, knees, and hips, although many people have flat arches without leg symptoms.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Possible effects
- The ankles, calves, knees, and hips may share altered loading during movement.
- Assessment
- Walking, heel raises, balance, strength, flexibility, and footwear can all provide useful clues.
How symptoms can develop
With a flexible flat foot, the arch lowers when you stand and may rise when you sit or lift the heel. The ankle can roll inward and the lower leg may rotate with it, changing the demands on the calf, shin, knee, or hip during walking. Some people notice tired legs, calf tightness, arch pain, foot pain, or pain after longer periods on their feet. Others have no discomfort and need no correction.
Symptoms can also come from a separate problem. For example, plantar fasciitis may cause heel or arch pain, while patellofemoral pain syndrome can cause pain around the kneecap. An examination looks at the whole movement chain, not simply the shape of the arch.
Ways to manage symptoms
Choose footwear that feels secure and allows your toes to move, then build walking, running, or standing time progressively. A clinician or physiotherapist may prescribe calf mobility, foot-strengthening, balance, hip-strengthening, and movement-control exercises. Custom orthotics can sometimes support comfort during activity, but they work best when selected for a specific symptom pattern and reviewed over time. Avoid forcing the arch into a painful position or changing several training factors at once. Keep a simple record of activity, footwear, and symptom response.
When to seek care
Book an assessment if leg or foot pain limits walking, lasts despite activity changes, or is steadily worsening. One-sided change in arch shape, new swelling, repeated falls, weakness, numbness, or inability to rise onto the toes should be examined. Seek urgent help after an injury with deformity or inability to bear weight, or when a joint becomes hot and swollen with fever. Children with pain, marked asymmetry, or difficulty keeping up with normal activity also merit review.
Questions for your doctor
Are my flat feet flexible, and are they linked to my leg symptoms? Which structure is most sensitive? What exercises and footwear changes should I try first? Would orthotics or physiotherapy be appropriate?

Questions people ask
Can flat feet cause tired legs?
They may contribute to fatigue in some people by changing muscle demands during standing and walking, but other causes should be considered if fatigue persists.
Do all flat feet need treatment?
No. Treatment is usually guided by pain, functional limits, progression, or a specific movement problem rather than arch shape alone.
