Flank pain without fever: what to know

A strained back or abdominal muscle may hurt with movement, coughing, or touch. Kidney stones can cause sudden, severe, wave-like pain and may cause nausea or blood in the urine without fever. Kidney infection commonly causes fever or chills but can begin with flank pain and urinary symptoms before these develop. A urinary tract infection may cause burning, urgency, or frequent urination, while flank pain raises concern for involvement beyond the bladder. Location alone cannot identify the source.

Note the pain's side, onset, movement triggers, severity, urine changes, nausea, and any temperature reading. Keep activity gentle and avoid heavy lifting if the pain followed exertion. Drink normally unless fluid restriction has been prescribed, and do not use someone else's antibiotics. Arrange a clinical review when pain persists, recurs, or comes with urinary symptoms, even if you have no fever.

Seek urgent help for severe or worsening pain, vomiting, faintness, confusion, inability to pass urine, or visible blood in the urine. Fever, chills, or marked weakness developing later increases concern for infection and needs prompt assessment. Pregnancy, kidney disease, or a single functioning kidney also calls for early medical advice.

Ask whether your symptoms warrant urine testing, blood tests, or imaging, and which pain relief is safe for you. Explain whether the pain changes with movement, whether urination has changed, and whether you have had stones, infections, injuries, or recent strenuous activity.
