Short answer

Flank pain when inhaling may reflect a pulled chest-wall muscle, but pain linked to breathing can also signal a lung or other urgent problem.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Breathing clue
Pain with cough or deep breath can point toward the chest wall or lung area
Urinary clue
Burning urination, frequent urination, or blood in urine suggests a urinary concern
Emergency clue
Severe breathlessness, chest pressure, fainting, or coughing blood

Why a breath may hurt

The muscles between the ribs and the lining around the lungs move as you breathe. Strain after coughing, exercise, or a blow can make a deep breath, cough, or twist painful. Pain may be local and tender to touch. A chest infection or inflammation around the lung can cause breathing pain with cough, fever, or shortness of breath. Kidney stones and kidney infection sit lower in the flank and may cause pain independently of breathing, with urinary symptoms or fever providing useful clues.

Describe whether the pain is sharp, pressure-like, or aching, and whether it changes with posture or coughing. Do not assume the location alone identifies the source.

What to do next

Rest from strenuous lifting and observe whether comfortable, normal breathing remains possible. Keep your posture supported and avoid repeatedly taking very deep breaths to test the pain. Seek a same-day clinical assessment if breathing pain is new, persistent, or accompanied by cough, fever, or worsening discomfort. A clinician may examine your chest and abdomen and decide whether urine or other tests are needed. Use medicines only as directed and consider your existing conditions before taking pain relief.

When to seek urgent help

Call emergency services for severe breathing difficulty, blue or grey lips, chest pressure, fainting, confusion, coughing blood, or sudden severe pain. Seek urgent care for flank pain with fever and chills, repeated vomiting, or a fast deterioration. After a chest or abdominal injury, prompt evaluation is wise if breathing is painful or limited.

Questions for your doctor

Ask whether the pain appears to come from the chest wall, lung area, or urinary system, and whether you need examination or testing. Tell the clinician about cough, fever, breathlessness, injury, urinary changes, recent travel, and medicines.

Questions people ask

Can a pulled muscle hurt when I inhale?

Yes. Rib and chest-wall muscles move during breathing, so strain can hurt with inhalation, coughing, or twisting.

Should I wait if breathing itself feels difficult?

No. New or worsening breathing difficulty needs urgent medical assessment, especially with chest pressure, fainting, or blue lips.
