Flank pain when I move: what should you know?

Short answer

Flank pain that starts or worsens when you move is often related to muscles or joints, but urinary causes can also produce pain and need attention when other symptoms appear.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Possible source
Muscle, joint, spine, rib, or urinary system
Useful detail
Whether touch or a specific movement reproduces the pain
Urgent warning
Fever, blood in urine, severe pain, or inability to pass urine

What movement-related flank pain can mean

A strained muscle between the ribs, abdominal wall, or back can hurt when you bend, reach, walk, or turn. Pain may be tender when touched and may follow lifting, exercise, coughing, or an awkward posture. Irritation from the lower spine or ribs can behave similarly. A kidney stone can cause flank pain that comes in waves and may spread toward the groin; a kidney infection can cause flank pain with fever, chills, nausea, or urinary discomfort. A urinary tract infection may cause burning or frequent urination, although flank pain with fever needs prompt assessment.

Note the exact location, whether the pain is constant or wave-like, and whether movement changes it. These details help separate a body-wall problem from pain linked to the urinary tract.

What you can do now

Reduce activities that clearly trigger the pain for a short period, but keep gentle movement if it does not increase symptoms. A warm compress may ease a muscle-related ache. Drink normally unless a clinician has advised fluid restriction, and do not force large amounts of fluid to treat unexplained pain. Avoid taking someone else’s prescription medicine. Arrange a clinical review if the pain persists, returns, or interferes with sleep or ordinary activity; assessment may include an examination and urine testing.

When to seek care

Seek urgent medical help for severe or rapidly worsening pain, fainting, confusion, repeated vomiting, inability to pass urine, blood in the urine, or pain with fever or chills. Get prompt advice after a significant injury, or if you are pregnant or have known kidney disease. Call emergency services for flank pain with chest pain, serious breathing difficulty, or new weakness.

Questions for your doctor

Ask what examination or urine tests may clarify the cause, whether your medicines could affect the kidneys, and which activities are safe while the pain settles. Mention any urinary changes, fever, injury, new exercise, or pain spreading toward the groin.

Questions people ask

Does movement-related flank pain always come from a muscle?

No. Movement can aggravate a muscle or joint problem, but kidney stones and urinary infections can also cause flank pain.

What should I record before an appointment?

Record when the pain began, its side and location, triggers, severity, urinary symptoms, fever, injury, and medicines taken.
