Flank pain radiating to the shoulder: what to know

The shoulder can feel pain referred from structures beneath the diaphragm, while a strained rib or back muscle may hurt along the side and upper body. Flank pain with fever, chills, nausea, or painful urination can occur with a kidney infection. Kidney stones more often cause severe cramping in the flank and may spread toward the groin, but symptoms vary. A urinary tract infection may cause burning, urgency, or cloudy urine and deserves review if flank pain develops.

Describe whether the shoulder pain is on the same side, whether deep breaths worsen it, and whether it began after an injury, meal, cough, or urinary change.

Stop strenuous activity and sit upright if that feels easier. Take ordinary fluids unless you have been advised otherwise, and avoid alcohol until the cause is clearer. Do not press firmly on a painful abdomen or use leftover antibiotics. A clinician can check your breathing, abdomen, back, and urine, then decide whether further assessment is needed.

If the discomfort is mild and clearly linked to movement, arrange routine advice if it does not settle. Keep a note of temperature, urinary symptoms, vomiting, and changes with breathing.

Call emergency services for chest pressure, severe breathlessness, fainting, blue lips, confusion, or pain with sweating and marked weakness. Seek urgent medical care for sudden severe abdominal or flank pain, fever with shaking chills, persistent vomiting, or blood in urine. Do not drive yourself if you feel faint or short of breath.

Ask whether the shoulder pattern suggests referred pain, a chest or abdominal issue, a urinary condition, or a muscle problem. Ask whether you need a urine test, examination, imaging, or a clear safety-net plan.
