Flank pain when lying down: what to consider

Lying on one side can load the back, ribs, or hip and make a strained muscle or irritated joint more noticeable. A different position can also change pressure on tissues around the abdomen. Kidney stones may cause waves of pain regardless of position, while kidney infection may cause steady flank discomfort with fever, chills, nausea, or urinary symptoms. A urinary tract infection usually causes lower urinary symptoms, but pain in the flank with fever suggests the infection may need prompt treatment. Reflux, bowel fullness, and nerve irritation can add positional symptoms.

Notice whether turning, bending, coughing, or pressing the area reproduces pain, and whether changing position eases it. Use supportive pillows and avoid lifting or twisting until a muscle strain is clearer. Record urine changes, temperature, nausea, bowel symptoms, and the pain's timing. Arrange a review if the symptom persists, disrupts sleep, or is not linked to a clear movement trigger.

Seek urgent care for severe or escalating pain, fever, chills, repeated vomiting, fainting, confusion, blood in the urine, or inability to pass urine. Immediate evaluation is also appropriate after a major injury or when pregnancy is possible and pain is one-sided or accompanied by bleeding or dizziness.
