Where flank pain is felt and what it can mean

Pain high in the side or back can involve the kidney, while pain that moves toward the lower abdomen or groin can occur with a kidney stone. Kidney infection may produce steady flank pain with fever, chills, nausea, or urinary symptoms. A urinary tract infection more often causes burning, urgency, or frequent urination, but pain higher in the back with fever needs prompt review. Pain that worsens with twisting, lifting, coughing, or touch can point toward muscles or the ribs. Nerves, bowel, and nearby pelvic structures may also refer pain to this area.

Use one finger to identify the most painful spot, then record whether the sensation is aching, sharp, cramping, or burning. Write down triggers, radiation, urinary changes, bowel symptoms, fever, rash, or injury. Rest from activities that clearly aggravate muscular pain, but do not ignore persistent or unexplained symptoms. A clinician may check the abdomen, back, and urine to distinguish urinary causes from body-wall pain.

Get urgent care for intense pain, fever or chills, repeated vomiting, faintness, confusion, inability to urinate, or blood in the urine. Seek immediate assessment for flank pain after a significant injury, or for new severe pain during pregnancy.
