Flank pain at night: what to know

Flank pain at night may reflect position or a urinary condition, and repeated sleep-disrupting pain deserves medical assessment. Pain after lying in one position may be muscular, while colicky pain, fever or urine changes suggest a problem that should be checked.

At night there are fewer distractions, and a mattress or sleeping position may load the back and side differently. A strained muscle often hurts with turning, reaching or pressing the area. Kidney stones may cause intense pain in waves that can travel toward the groin. Kidney infection may cause steady flank pain with fever, chills, nausea or urinary discomfort. A urinary tract infection can cause urgency or burning and may disturb sleep. The time of day alone cannot establish the cause.

Write down when the pain wakes you, your sleeping position, movement triggers, temperature and any urine changes. Try a comfortable supported position and avoid heavy exertion while you observe a mild, movement-related ache. Keep normal hydration unless a clinician has given different advice. Book an appointment if the pain continues over several nights, becomes more intense or is joined by urinary symptoms.

Get urgent care for severe or rapidly worsening pain, fever or chills, repeated vomiting, visible blood in urine, inability to pass urine, faintness or confusion. Seek prompt help after an injury if pain is severe or breathing is affected.
