Flank pain and gas: what to know

Gas may cause shifting flank discomfort, but persistent or severe flank pain needs assessment for urinary and other causes. Bloating, belching and relief after passing wind support a digestive explanation, while fever, urine changes or wave-like pain point toward the urinary system.

Gas discomfort often changes with meals, bowel movements or body position and may be felt across the abdomen or under the ribs. Constipation can add pressure. Kidney stones may cause intense pain that moves toward the groin, with nausea or blood in urine. Kidney infection can cause steady flank pain with fever, chills, vomiting or urinary burning. A urinary tract infection may cause frequency or urgency even without flank pain. A clinician may examine your abdomen and back and test urine when urinary features are present.

Notice whether symptoms follow particular foods, constipation, movement or urination. Gentle walking and regular meals may help trapped gas; avoid foods that clearly trigger bloating for a short period. Drink normally and do not use antibiotics without advice. Book an appointment if pain continues, returns often, or is not clearly relieved by passing wind or opening your bowels.

Get urgent care for severe or worsening pain, a hard or very swollen abdomen, repeated vomiting, inability to pass stool or gas, fever with flank pain, faintness, or blood in urine or stool. Prompt review is also needed if pain follows an injury or you feel unusually weak.
