Flank and Side Pain: How to Think About It

Side pain may be felt beneath the ribs, toward the back, or between the waist and hip. A sore spot that changes with bending, turning or touch is often approached differently from deep, cramping pain. Kidney stones can cause intense waves that move downward, while a kidney infection can cause flank discomfort with fever, chills or feeling unwell. Urinary burning, urgency or frequent urination may accompany a urinary tract infection. Pain worsened by a breath or following an impact may need assessment for a rib or chest cause.

Write down the start time, exact side, severity, triggers and any change in urine, bowel movements or breathing. Pause heavy exercise and choose gentle movement only if it does not increase pain. Drink your usual fluids unless you have been told otherwise, and do not self-start antibiotics. A clinician may check your abdomen, back, temperature and urine before deciding whether further tests are useful.

Get urgent medical help if pain is sudden and severe, follows a significant injury, or comes with breathlessness, fainting, confusion, fever with shaking chills, persistent vomiting or blood in the urine. Arrange prompt review when symptoms keep returning or interfere with normal activity even without these warning signs.
