Scalp Flakes That Are Not Dandruff

A scalp may shed scale after contact with hair dye, fragrance, oils, gels, or a harsh cleanser. Seborrheic dermatitis can produce itchy, inflamed skin with greasy scale, and psoriasis can create thicker, more adherent plaques that extend beyond the hairline. Scalp ringworm is more concerning when flaking comes with broken hairs, tender patches, or hair loss. A flaky scalp can also be mistaken for residue from dry shampoo or styling products.

The symptom does not prove that dandruff is absent or present. A clinician may examine the scale, skin, and hair, and may test material from the scalp when infection is suspected.

Keep a short record of products, washing frequency, itch, redness, and hair changes. Wash gently, rinse fully, and avoid picking. Stop a recently added product while the skin settles. Medicated shampoo treatment may help some causes, while topical antifungal treatment is used only when a clinician identifies a fungal problem; these are not interchangeable. Avoid sharing hair tools, hats, or towels until an infectious cause has been excluded.

Book an appointment if flakes persist despite gentle care, recur often, or are accompanied by significant itch, redness, pain, crusting, oozing, swollen glands, or hair loss. Seek prompt assessment for a tender expanding patch or fever.
