What the bleeding may mean

After pregnancy, ovulation and the uterine lining may take time to settle into a pattern. The first true period can therefore feel different from earlier cycles. Bleeding after birth is not always a period, so timing matters: postpartum bleeding begins after delivery and gradually changes, while a period follows the return of the menstrual cycle. Hormonal contraception, breastfeeding, and an irregular cycle can all affect what you notice.

Heavy menstrual bleeding can lead to iron deficiency anaemia, especially when tiredness, breathlessness, dizziness, or paleness develop. Fibroids, adenomyosis, polyps, changes in the uterine lining, polycystic ovary syndrome, or perimenopause may also contribute, although a heavy first postpartum period does not identify any one condition.

What to do now

Record when the bleeding began, how quickly pads become soaked, whether clots are present, and any pain, fever, weakness, or unpleasant-smelling discharge. Use external pads while the amount is unclear, rest, drink fluids, and arrange a review with a gynaecologist or maternity team if the flow is much heavier than expected or does not ease. Do not start iron tablets or bleeding medicines without checking which options fit breastfeeding, other medicines, and your medical history.

When to seek urgent care

Seek urgent medical help if bleeding is rapidly soaking pads, you feel faint or confused, have severe or worsening pelvic pain, a fast heartbeat, marked weakness, fever, or trouble breathing. Call emergency services if you might collapse. These signs can indicate significant blood loss or infection and should not be judged by how common heavy periods are.
