First period after Nexplanon

The first period after Nexplanon may be irregular, lighter, heavier, longer, or absent because the implant changes the hormone pattern controlling the uterine lining. After removal, cycles may also take time to resemble your previous pattern, and ovulation can return before the first period.

Nexplanon, an etonogestrel implant rod, releases a progestogen. This can make bleeding unpredictable rather than producing a neat monthly cycle. Spotting, bleeding for longer than usual, or no bleed can all occur. Bleeding changes do not reliably show whether contraception is working, so check that the rod was inserted correctly, remains within its approved use period, and has not been affected by medicines your clinician has identified.

If the implant was removed, pregnancy is possible soon afterward. A late or unusual bleed with pregnancy symptoms should prompt a test, particularly if there was unprotected sex.

Keep a simple record of bleeding days, flow, cramps, and any missed or late doses of other medicines. Check the implant site for a new change in position, redness, swelling, or discharge, and arrange a review if you cannot feel the rod as instructed. Do not remove or manipulate it yourself. Discuss alternatives such as a hormonal coil if the pattern is difficult to manage.

Seek prompt medical advice for very heavy bleeding, severe lower abdominal pain, faintness, or a positive pregnancy test with pain or bleeding. Get help for possible infection at the insertion site, including spreading redness, warmth, swelling, pus, or fever. If bleeding remains disruptive, a clinician can check for pregnancy, review medicines, and discuss treatment or another contraceptive method.

Ask whether your pattern is expected for the implant, when a pregnancy test is needed, and which medicines may reduce contraceptive effectiveness. Clarify the rod’s replacement or removal date, what to do if you cannot feel it, and whether a hormonal coil suits your health and preferences.
