First period after delivery with heavy flow

Short answer
The first period after delivery may be heavier or irregular, but heavy flow with faintness, breathlessness, severe pain, or fever needs prompt medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Before calling it a period
- Consider whether postpartum bleeding had fully stopped.
- Track
- Record pad changes, clots, pain, and whether flow is easing.
- Urgent signs
- Faintness, breathlessness, severe pain, fever, or rapidly heavy flow need urgent help.
What heavy flow can mean
The first menstrual cycle after birth may differ because ovulation and the uterine lining are adjusting; breastfeeding can delay the return of periods. First, consider whether bleeding is actually lochia, which is postpartum bleeding that gradually changes and lessens. Heavy Periods can also occur with uterine fibroids, adenomyosis, endometrial polyps, or endometrial hyperplasia. Repeated heavy bleeding may lead to Iron Deficiency Anaemia, causing tiredness, headaches, paleness, or breathlessness on exertion.
How to monitor and prepare
Note when bleeding began, how often you change pads, whether clots are present, and whether the flow is easing. Use pads while the pattern is unclear, particularly if you are still in the postnatal recovery period. Rest and drink fluids, and include iron-rich foods. Tell a clinician about your delivery, breastfeeding, contraception, pain, and any chance of pregnancy. A repeat pattern or symptoms of anaemia merits a review rather than self-treatment alone.
When to seek medical help
Get urgent help if bleeding is rapidly soaking products, you feel faint or confused, have shortness of breath or chest discomfort, severe abdominal pain, fever, or feel suddenly very unwell. Contact your maternity or gynaecology team promptly if heavy flow persists, keeps returning, or is much heavier than your previous periods. The urgency depends on symptoms and the speed of bleeding, not on whether heavy periods are common.
Questions for your doctor
Ask whether the bleeding is lochia or menstruation, whether you need a blood count or iron assessment, and whether pelvic examination or ultrasound is appropriate. Ask which treatments are compatible with breastfeeding and contraception, and when to arrange follow-up if the next cycle is also heavy.
