Finerenone and heart failure: what should you know?

Finerenone blocks a hormone pathway that can promote inflammation and scarring in the heart and kidneys. It is not a water tablet and does not replace the rest of a heart-failure plan. Suitability depends on the type of heart failure, kidney function, potassium level and other medicines. Treatment decisions may also account for hypertension or atrial fibrillation. Symptoms such as fatigue, cough, palpitations or swollen ankles can have several causes and should be interpreted clinically.

Tell the prescriber about kidney disease, potassium supplements, salt substitutes and every prescription or non-prescription medicine. Do not borrow tablets. Keep a record of swelling, breathlessness, weight changes and dizziness to discuss at review.

Contact your care team promptly for worsening breathlessness, rapidly increasing swelling, faintness, severe weakness or a new irregular heartbeat. Seek emergency help for severe breathing difficulty, chest pressure, collapse or sudden confusion. Do not wait for a routine blood test if you feel acutely unwell.
