Fibroid bleeding after sex: what to know

A fibroid is a non-cancerous growth in the muscle of the womb. Depending on its position, it can alter the shape of the womb, irritate nearby tissue, or contribute to bleeding at times outside a period. Bleeding after sex can also come from a sensitive cervix, vaginal dryness, inflammation, or an infection such as Chlamydia. Less common but serious causes include Cervical Cancer or Vaginal Cancer. The amount, colour, timing, pain, pregnancy possibility, and whether the bleeding repeats all help guide assessment.

Arrange an appointment with a gynaecologist or primary-care clinician, particularly if the bleeding has happened more than once. Note when it occurs, how long it lasts, whether intercourse was painful, and any discharge or pelvic pain. The clinician may examine the vagina and cervix, review screening history, test for infection, and arrange an ultrasound when a womb-related cause is suspected. Avoid inserting anything into the vagina until heavy or painful bleeding has settled.

Seek urgent help for heavy bleeding, faintness, severe or worsening pelvic pain, fever, or bleeding during a possible pregnancy. Prompt assessment is also appropriate for bleeding after sex that continues, occurs after menopause, or comes with unusual discharge or a new pelvic symptom. These signs do not identify one diagnosis; they indicate that examination should not be delayed.

Ask whether the bleeding could relate to the location of a fibroid, whether you need cervical screening or infection tests, and which examination or scan is suitable. You can also ask how treatment would change if the cause were vaginal dryness, cervical dysplasia, or a fibroid.
