Fever, headache and rash: what to know

An infection can trigger fever while inflammation causes headache and changes in the skin. Viral illnesses such as COVID-19 or a common cold may cause overlapping symptoms, although a rash is not present in every case. Dengue fever is another possibility in someone who has recently been in an area where it occurs. Dehydration and heat exhaustion can worsen headache and make you feel weak, especially after sweating or spending time in heat. Allergic rhinitis and chronic sinusitis can cause head or facial discomfort, but fever or a widespread rash needs a broader assessment. Generalised anxiety disorder does not explain every fever, so do not assume stress is the cause.

Rest in a cool, comfortable place and drink fluids regularly if you can swallow normally. Record your temperature, when the headache began, where the rash started, and whether it is spreading, itchy, painful or bruised-looking. Avoid sharing towels, cups or close personal items until an infectious cause is clearer. Use only fever or pain medicine that is suitable for you and follow its label directions; ask a pharmacist if you have liver, kidney or stomach problems, take other medicines, or are caring for a child. Do not scratch the rash.

Seek urgent care now for a stiff neck, confusion, fainting, trouble breathing, severe weakness, repeated vomiting, a rapidly spreading rash, or spots that do not fade when pressed. Get prompt medical advice for a severe or rapidly worsening headache, fever after travel, or symptoms in a young infant, older adult, pregnant person or someone with reduced immunity.

Ask which examinations or tests could distinguish a viral illness from dengue fever or another infection. Tell the clinician about recent travel, insect bites, new medicines, sick contacts, heat exposure and fluid intake. Ask how to monitor the rash and which fever medicines are appropriate for you.
