Fever and chills after giving birth

Fever and chills postpartum deserve prompt attention because infection can involve the uterus, wound, breasts, urine or lungs. Brief shivering can occur around the physical stress of birth, but ongoing fever, worsening pain or feeling acutely unwell should be assessed.

Lower abdominal tenderness, worsening bleeding or foul-smelling discharge can point to a uterine problem. Redness, swelling, pus or increasing pain around a tear, incision or caesarean wound needs review. Breast redness and a painful area may accompany a breast infection. Burning urine or flank pain can occur with a Urinary Tract Infection or Kidney Infection. Cough and breathlessness may reflect Pneumonia; Influenza can affect anyone after delivery.

Measure your temperature, note the time and contact your maternity team or clinician for advice. Drink fluids, rest when possible and inspect any wound without pressing it. Keep a record of bleeding, discharge, breast changes, urinary symptoms and pain. Ask before taking medicines, particularly if you are breastfeeding or have been prescribed other treatment.

Seek emergency care for trouble breathing, chest pain, confusion, fainting, seizure, blue lips, severe weakness, uncontrolled bleeding or rapidly worsening illness. Fever with severe abdominal pain, a spreading wound redness, inability to drink, or a sudden change in alertness should not wait for a routine appointment. Sepsis can progress quickly after infection, so marked deterioration needs immediate help.

Ask what examination is needed, whether the uterus, wound, breasts or urine should be checked, and which fever medicines are compatible with breastfeeding.
