Fever after shivering: what it can mean

Chills or shivering can occur when the brain temporarily sets the body’s temperature higher. Muscles contract to produce heat, so you may feel cold before the fever is obvious. Viral illnesses such as influenza can cause this pattern, while a urinary infection, kidney infection or pneumonia may produce fever with pain, cough or breathing symptoms. Malaria and dengue fever are important possibilities after relevant travel or mosquito exposure.

Rest, drink fluids regularly and use a light blanket until the shaking settles. Avoid ice baths and heavy wrapping, which can worsen discomfort. Measure the temperature with a reliable device and write down when the fever and chills occur. Take paracetamol or another recommended fever medicine only according to the label or a clinician’s advice. Do not start leftover antibiotics.

Call emergency services for confusion, fainting, severe breathlessness, blue lips, a seizure, or skin that is cold and mottled with extreme weakness. Get urgent medical assessment for a rapidly worsening illness, severe headache with a stiff neck, repeated vomiting, very little urine, or fever after travel to a malaria-risk area. Fever with a painful cough, flank pain or burning urine also warrants prompt review.

Tell the clinician when the shaking began, the highest measured temperature, recent travel, medicines taken and any cough, urinary symptoms, rash or pain. Ask which tests may identify the source and how long to monitor symptoms.
