Feet and legs swollen after a C-section

Short answer
Feet and legs can swell after a C-section as your body shifts fluid after pregnancy and surgery, but one-sided swelling or other warning symptoms needs prompt medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common pattern
- Swelling in both feet and legs that eases with elevation can occur after birth.
- Do not ignore
- One-sided painful swelling, chest symptoms, or severe headache needs prompt assessment.
What the swelling can mean
Swelling in both feet and legs is often related to extra fluid retained during pregnancy, fluids given around surgery, reduced movement, and the gradual return of circulation to its usual pattern. It may feel tight by the end of the day and improve when your legs are raised. The skin may leave a temporary indentation after gentle pressure.
Swelling that is clearly worse on one side, especially with calf tenderness, warmth, redness, or pain, has a different pattern and should not be explained away as routine recovery. Headache, vision changes, upper abdominal pain, breathlessness, or a sudden increase in swelling can also signal a problem after birth, including pre-eclampsia.
What you can do at home
Take short, gentle walks as advised by your maternity team and avoid staying in one position for long periods. When resting, place your legs on a supported cushion without putting pressure on your incision. Drink normally, wear comfortable footwear, and ask before using compression socks, particularly if one leg is painful or discoloured. Do not massage a painful swollen calf.
Follow your discharge instructions for wound care and activity. Postnatal Physiotherapy may help you return to movement safely after a Caesarean Section, especially if discomfort is making it difficult to walk.
When to seek medical care
Contact your maternity unit or clinician promptly if swelling is sudden, rapidly worsening, or mainly affects one leg, or if you have calf pain, redness, warmth, or difficulty walking. Seek urgent help for chest pain, sudden breathlessness, coughing blood, fainting, severe headache, new vision changes, or pain beneath the ribs. These symptoms need assessment rather than home monitoring.
If swelling is mild and affects both legs, mention it at your postnatal check, particularly if it is not settling or is accompanied by raised blood pressure symptoms. A clinician can check your blood pressure, circulation, wound, and other causes.
Questions to ask your clinician
Ask whether the pattern and timing of your swelling fit normal postnatal fluid changes, whether you need a blood-pressure check, and which activities are suitable while your incision heals. Tell them about medicines, intravenous fluids, pain, reduced mobility, or swelling that changes during the day.

Questions people ask
How long can swelling last after a C-section?
Postnatal swelling often improves gradually as excess fluid leaves the body. Contact your clinician if it is worsening, persistent, or accompanied by other symptoms.
Can I walk with swollen legs after a C-section?
Gentle walking is often part of recovery when your maternity team agrees, but stop and seek advice if walking causes marked pain, breathlessness, or one-sided calf symptoms.
