Feeling chills without a fever: what should you know?

Shivering is the body’s way of generating heat, but chills can also occur during an adrenaline surge, after becoming chilled, with low blood sugar, or as a medicine effect. An infection may be starting before a fever appears or may not produce a measured fever. Influenza, pneumonia, a urinary tract infection, kidney infection, malaria, dengue fever, and sepsis become more concerning when chills occur with respiratory, urinary, abdominal, skin, or whole-body symptoms. Recent travel, mosquito exposure, and immune problems change the level of concern.

Move to a comfortably warm place, use light layers, drink fluids, and check your temperature with a reliable thermometer if you feel unwell. Rest and record when the chills occur, how long they last, and any cough, pain, urinary change, rash, diarrhoea, or travel history. Do not use leftover antibiotics. Contact a clinician if episodes recur, follow travel to a malaria area, or come with signs of infection even when the thermometer reading is normal.

Seek emergency help for confusion, extreme weakness, difficulty breathing, blue or clammy skin, fainting, severe pain, or rapidly worsening illness. Urgent assessment is needed for shaking chills with a fast heartbeat, very little urine, persistent vomiting, a new rash, or suspected malaria after travel. Prompt review is also sensible for older adults, infants, pregnant people, or anyone with reduced immunity and new chills.
