What should you know if you feel a lump in your neck?

A lump may be a swollen lymph node responding to an infection, a thyroid nodule or enlargement, a salivary-gland problem, a cyst, or another tissue change. Head and neck cancer can present with a neck lump, as can lymphoma, but a neck lump does not by itself establish either diagnosis. A cancer of unknown primary may also be considered when a lymph node contains cancer cells but the starting site is not yet found.

The clinician will consider the lump’s location, size, firmness, mobility and duration, along with symptoms such as sore throat, voice change, swallowing difficulty, fever or weight change. Assessment may include examination, imaging or a needle sample.

Arrange a medical assessment if the lump persists, grows, feels hard or fixed, returns repeatedly, or has no clear explanation. Note when you first noticed it, whether it changes with illness, and any associated symptoms. Do not repeatedly squeeze or massage the area. Bring a list of medicines and mention recent infections, dental problems, smoking or tobacco exposure, and thyroid history.

A clinician may examine your mouth, throat, thyroid and lymph-node areas before deciding whether an ultrasound, other imaging or tissue sampling is appropriate.

Seek urgent help if the lump is accompanied by difficulty breathing, inability to swallow fluids, rapidly increasing swelling, severe weakness, or swelling of the lips or tongue. Arrange prompt medical review for persistent hoarseness, coughing blood, unexplained night sweats, or a lump that continues to enlarge. These warning signs need assessment because they can reflect airway problems or a condition requiring timely treatment.

Ask where the lump appears to arise, what examination or imaging is recommended, whether a needle sample is needed, and how long results may take. Ask which changes should prompt earlier review and whether an ear, nose and throat, endocrine or cancer specialist should be involved.
