Febrile seizure with tonic-clonic movements

Tonic means the muscles stiffen; clonic describes repeated, rhythmic jerking. During a fever-related event, a child may lose awareness, fall, have their eyes turn upward, or briefly change colour around the lips. The movements can look dramatic, but their appearance alone cannot confirm the cause.

A clinician considers the fever, the child’s age and health, the length of the episode, and recovery. Seizures without fever, prolonged events, or movements limited to one side require particular attention because they may not fit a simple febrile pattern.

Lay the child on the floor or another safe surface and move hard objects away. Cushion the head, remove glasses, loosen clothing around the neck, and note the time. Do not hold the arms or legs still, put anything between the teeth, or offer water or medicine during the movements.

When the jerking ends, place the child on their side and watch their breathing. Stay until awareness returns, keep them warm without overheating, and tell the healthcare professional exactly what you saw. A video can help if it is safe to record without delaying care.

Call emergency services when the seizure reaches five minutes, starts again before recovery, causes breathing difficulty, happens in water, or results in a significant injury. Get urgent assessment for a first event, a stiff neck, severe headache, a non-fading rash, persistent confusion, or continued weakness. These features can occur with serious illness, including meningitis.

Ask whether the episode’s duration and movement pattern fit a febrile seizure, how to manage future events, and whether a rescue medicine plan is appropriate.
