Fatty Liver and Dark Urine: What to Know

Concentrated urine from not drinking enough, sweating or heat exposure can look darker. Medicines, foods, blood and liver or bile problems can also change its colour. Fatty liver may be found on imaging or blood tests without causing obvious symptoms, so urine colour alone cannot show whether it has worsened.

Dark urine with yellow skin or eyes, pale stools, itching, nausea, upper abdominal discomfort or unusual tiredness can suggest a liver or bile-flow problem. Fever, vomiting or recent exposure to infection may point to another cause such as viral hepatitis. A clinician needs the full history rather than assuming fatty liver is responsible.

Notice whether the colour improves after drinking fluids and whether it returns. Avoid alcohol until a clinician has reviewed unexplained dark urine, and do not stop prescribed medicines without advice. Record the colour, onset, fluid intake, pain, bowel changes and any new medicines or supplements.

Arrange a medical review if the change persists or recurs. Assessment may include urine testing, liver blood tests and imaging depending on your symptoms. Bring a list of medicines and supplements, and mention recent travel, heat exposure or possible infection.

Seek urgent care for dark urine with yellowing, severe abdominal pain, confusion, fainting, repeated vomiting, high fever, very little urine or inability to drink. Contact a clinician promptly if the urine remains dark despite good hydration, or if you notice pale stools, marked itching or worsening fatigue.
