What the symptoms may indicate

Chest tightness can feel like pressure, constriction, difficulty taking a full breath, or discomfort across the chest. Fatigue may follow poor sleep, stress, a respiratory infection, or reduced oxygen exchange. Anaemia can contribute to tiredness and breathlessness. Anxiety or panic can tighten chest muscles and change breathing, but those symptoms should not be assumed to be the cause without considering other possibilities. Heart and lung problems can also present with chest discomfort and fatigue, particularly when symptoms occur with exertion, spread to the arm, back, neck, or jaw, or come with sweating, nausea, or dizziness.

Stop strenuous activity and sit upright while you assess how you feel. Note when the tightness began, what brings it on, how long it lasts, and whether breathing, movement, meals, or stress affect it. Do not drive yourself if symptoms are severe or worsening. Arrange a same-day clinical review for new, recurring, or unexplained symptoms, even if they settle. A clinician may check your heart, lungs, oxygen level, blood pressure, and relevant blood tests.

When to seek urgent care

Call emergency services now for severe or persistent chest pressure, difficulty breathing, fainting, blue or grey lips, confusion, or sudden weakness. Urgent help is also needed when discomfort spreads to the arm, back, neck, or jaw, or occurs with cold sweating, nausea, or a racing heartbeat. Do not wait to see whether rest makes these warning signs disappear.
