What should you know about fat harvesting for breast reconstruction?

The surgeon removes fat with liposuction from an area such as the abdomen, hips, thighs, or another suitable site. The fat is processed and carefully injected into the breast area to improve contour, add fullness, or soften an edge around an implant. It may be used after mastectomy and can be combined with implant-based or flap reconstruction.

Some transferred fat may not remain, so more than one procedure can be considered. The donor area has its own bruising, swelling, and contour changes. Your surgeon will assess available tissue, previous treatments, skin condition, and the shape you hope to achieve.

Discuss whether fat grafting is suitable alone or as part of a staged plan. Tell the team about previous breast surgery, radiotherapy, smoking, medicines, and conditions that affect healing. Ask where fat will be taken, how the breast will be supported, and whether an implant or tissue expander is also planned.

After surgery, follow instructions for dressings, activity, compression garments, and pain relief. Bruising and swelling can affect the early shape. Attend follow-up visits so the surgeon can check both the breast and donor area.

Contact your surgical team for increasing redness, warmth, drainage, fever, worsening pain, or a sudden change in swelling. Seek urgent help for chest pain, breathing difficulty, fainting, or heavy bleeding. A firm area or lump during healing should be reported so it can be examined rather than guessed at.

Ask what result is realistic for your anatomy, whether staged grafting is likely, how scars and donor-site changes will look, and how this plan fits with cancer treatment. Ask when screening or imaging can resume if a new breast lump develops.
