What is breast reconstruction?

Breast reconstruction reshapes the breast area after cancer surgery using an implant, a tissue flap, or a staged combination of methods.

It may be performed at the same operation as a [Mastectomy](/treatments/surgical-oncology/mastectomy/) or planned later. Reconstruction can restore shape and symmetry, but it does not recreate a natural breast in every way and it does not treat the cancer itself.

What does treatment involve?

The surgeon discusses timing, chest-wall skin, available body tissue, possible nipple and areola procedures, and whether radiation or other cancer treatment is planned. Implant reconstruction may use a temporary [Tissue Expander](/materials/tissue-expander/) before a permanent [Breast Implant](/materials/breast-implant/). Flap reconstruction uses tissue moved from another part of the body. Some plans require more than one operation.

What are the side effects, and which are serious?

Expected effects include swelling, bruising, tightness, altered sensation, scars and tiredness. Implant or expander problems can include infection, fluid collection, wound opening, movement or capsular tightening. Flap surgery can affect the donor area and rarely compromise the transferred tissue. Fever, worsening redness, drainage, sudden swelling, severe pain, breathing difficulty or a cool or discoloured breast needs prompt medical contact.

What does the evidence say about outcomes?

Outcomes vary with the operation, healing, body shape, previous treatment and whether radiation is needed. Reconstruction can improve clothing fit and body confidence for some people, while others prefer a flat chest or an external prosthesis. The surgeon should explain likely shape, sensation, scars, symmetry and the possibility of revision rather than promise an identical result.

Reconstruction is optional. Choosing it later, choosing no reconstruction or using an external prosthesis are all valid paths. A reconstructive plan should fit cancer treatment timing, general health, recovery support and personal preferences.

Who may consider reconstruction?

People who have breast tissue removed may discuss reconstruction if they want a rebuilt contour. Suitability depends on the chest tissues, prior or planned radiation, smoking, medical conditions, body tissue and ability to complete follow-up. Surgery is followed by wound care, monitoring, drain management when needed, and later procedures for shaping, symmetry or nipple reconstruction if desired.

Recovery and follow-up

Early recovery involves restricted lifting, pain control, wound checks and gradual return to movement. Flap procedures also involve healing at the donor site. Recovery length differs by technique and by whether reconstruction is staged. Attend follow-up visits even when healing appears smooth, because the team checks the breast and donor area over time.

Risks to discuss

Risks include bleeding, infection, delayed wound healing, fluid collection, changes in sensation, asymmetry and the need for further surgery. Radiation can affect healing and implant or flap results. Ask which symptoms require urgent review and how complications would be managed.

Alternatives to reconstruction

You may choose no reconstruction, an external breast prosthesis, specialised clothing or delayed reconstruction. These options can be revisited after cancer treatment. A decision is not a test of courage; it should reflect comfort, health, timing and how you want your body to look and feel.

Ask for a joint discussion with the breast and plastic surgery teams about cancer treatment, radiation, scars, sensation, staged operations and recovery support. Request clear instructions for drains, exercise and warning signs before leaving hospital.

Can reconstruction be done during mastectomy?

Yes, immediate reconstruction may be possible, but delayed reconstruction can also be planned when later cancer treatment or personal circumstances make that more suitable.

Will reconstruction restore normal sensation?

Sensation often changes after breast surgery and may remain reduced or different; the expected pattern depends on the operation and nerve healing.

Can an implant be used after an expander?

Often, an expander can gradually prepare the tissues before placement of an implant, but the exact sequence depends on healing and the surgical plan.
