Short answer

Exercises for ac joint pain should be gentle, keep the shoulder within a comfortable range, and stop if pain or swelling increases.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What the joint does

The acromioclavicular joint sits where the collarbone meets the top of the shoulder blade. Pain there can follow a fall, repeated overhead work, lifting, or irritation in the joint. Pain with reaching across the body or lifting the arm can help a clinician locate the problem, but it does not establish the cause by itself.

Widespread pain or fatigue may point to a broader problem such as Fibromyalgia or Hypothyroidism, while a hot, swollen joint needs assessment for inflammatory or infectious causes. Gout, Dengue Fever, Influenza, Brucellosis, Long COVID, and Obesity may be relevant only when other symptoms and medical history support them.

Gentle movement

Start with relaxed shoulder rolls, pendulum movements with the arm supported by gravity, and slow shoulder-blade setting. Keep the elbow close to the body for light strengthening, and progress only when daily movements are easier. Avoid deep cross-body stretches, heavy pressing, and repeated overhead lifting while they reproduce sharp pain.

Use a gradual routine rather than forcing range. Ice or heat may feel helpful for short periods; protect the skin and stop if symptoms worsen. A physiotherapist can adapt exercises after an injury and check whether the joint needs support or imaging.

When to seek care

Seek prompt assessment after a fall with deformity, inability to lift the arm, major swelling, numbness, or severe pain. Arrange review for fever, redness, a hot joint, unexplained weight change, persistent night pain, or symptoms that are not settling. Emergency help is needed for chest pain or breathlessness with shoulder pain.

Questions for your doctor

Ask whether the ac joint is the pain source, which movements should be limited, when strengthening can begin, and whether physiotherapy or imaging is appropriate.

Questions people ask

Can exercise make ac joint pain worse?

Yes. Heavy or repeated overhead loading can irritate the joint, especially after an injury or while swelling is present.

What is a safe starting exercise?

A supported pendulum or gentle shoulder roll is often a low-load starting point, provided it does not increase pain.
