Excessive sweating on the back

Short answer
Excessive sweating on the back may reflect heat or activity, but recurring heavy sweating can need assessment for an underlying cause.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Check
- Clothing, equipment, heat, activity, rash, and whether other areas sweat
- Protect skin
- Change damp clothing, cleanse gently, dry carefully, and reduce friction
Why the back may sweat heavily
The back contains many sweat glands and can become damp under tight clothing, backpacks, sports equipment, or non-breathable fabrics. Sweat can also collect where skin touches clothing for long periods, increasing rubbing and irritation. Compare the back with other areas: sweating only during heat or exertion suggests a different pattern from sudden episodes while resting or sleeping.
Generalised sweating can accompany hyperthyroidism, hypoglycaemia, obstructive sleep apnoea, infection, or medicine effects. Back sweating alone cannot tell you which explanation applies. A clinician may ask about itching, rash, fever, palpitations, snoring, thirst, and recent changes in health or treatment.
Ways to reduce discomfort
Choose loose, breathable tops, change damp clothing promptly, and wash gently after heavy sweating. Pat the skin dry rather than scrubbing it, and seek advice if a rash becomes painful, cracked, or persistent. Write down triggers such as heat, movement, stress, meals, and medicines; this helps separate local triggers from whole-body symptoms.
If sweating remains troublesome, arrange a review. Management may involve treating a medical cause, choosing a suitable topical product, or considering specialist options. Iontophoresis, Botox For Sweating, and Microwave Sweat Gland Treatment are different approaches and are not appropriate for every body area or person.
When it needs medical attention
Seek urgent care for sweating with fainting, chest pain, severe breathlessness, confusion, or signs of heat exhaustion. Arrange prompt assessment for night soaking, persistent fever, unexplained weight loss, new weakness, or a rapidly changing rash.
Questions to bring
Ask whether the distribution and timing suggest a local trigger or generalised sweating, whether your medicines could contribute, and how to protect the skin from friction. If treatment is discussed, ask how the back would be treated and what temporary skin effects to expect.

Questions people ask
Why does my back sweat through my shirt?
Heat, exertion, occlusive fabric, pressure from equipment, stress, and increased sweat production can all contribute; the pattern matters more than the shirt alone.
Can back sweating cause a rash?
Moisture and friction can irritate skin and encourage a rash, so persistent, painful, or changing skin should be assessed.
