Short answer

ESPEN sarcopenia guidance describes a condition involving reduced muscle strength with changes in muscle quantity or quality, requiring assessment and an individual care plan.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Main feature
Reduced muscle strength, with changes in muscle quantity or quality.
Daily effects
Standing, walking, carrying, and balance may become harder.
Care focus
Strength, nutrition, mobility, and fall prevention are reviewed together.

How sarcopenia affects daily life

Sarcopenia can make standing from a chair, climbing steps, carrying groceries, or walking outdoors more difficult. Muscle weakness is the key concern, while reduced muscle quantity or quality can add to loss of reserve. Ageing, inactivity, illness, hospital stays, poor nutrition, and other health conditions may contribute. Muscle cramps are not specific to sarcopenia, and difficulty walking can have causes involving joints, nerves, balance, or the heart and lungs. Assessment commonly combines strength testing with a review of movement, nutrition, health conditions, and sometimes body composition.

Building a safer plan

Ask a clinician or physiotherapist to assess strength before starting a new programme. Progressive resistance exercise may help the muscles adapt, while balance and mobility work can support everyday function. A nutrition professional may review protein and energy intake, swallowing, appetite, and medicines that affect eating. Exercise Therapy or Strength Training Rehabilitation can be adapted to your ability, pain, equipment, and home environment. If you have unsteadiness or previous falls, a Falls Prevention Programme can add lighting, footwear, transfer, and balance strategies. Progress should be checked by function, not appearance alone.

When to seek care

Seek urgent care for sudden weakness, new one-sided weakness, a fall with injury, chest pain, severe breathlessness, or inability to stand. Arrange a prompt review if walking is deteriorating, you are falling, weight or appetite is changing, or fatigue is preventing basic activities. A rapid change may signal an illness, medication effect, or neurological problem that needs separate assessment.

Questions for your doctor

How will you assess my muscle strength and muscle quantity? Could another condition or medicine be contributing? Which exercises are safe for me, and how should they progress? Do I need nutrition support or a falls assessment? What change should prompt a review?

Questions people ask

Is sarcopenia only about having smaller muscles?

No. Strength and physical function are central to assessment; muscle quantity or quality is considered alongside them.

Can exercise help with sarcopenia?

A tailored resistance and mobility programme may improve strength and function. It should be matched to your health and starting ability.
